Independent Contractor & External Key Personnel Questionnaire
In recent updates, the following question has been added to this form and is used to assist in identifying an Independent Contractor:
- “Is the person going to be an author on any public dissemination such as a manuscript, paper, presentation, abstract, etc. coming from this research project?”
Outgoing Subrecipient Letter of Intent Form
To be in compliance with the U.S. Department of Homeland Security (DHS) requirements for Civil Rights Evaluations, we have added an additional section to this form. This section must be completed by the subrecipient if our Prime Sponsor is U.S. DHS.
