Purpose

For use by UNC faculty, students and staff to submit invention disclosures to OTD.

If not downloading the PDF file, refer to the following:

1. Click the Report form that matches your word processor,
   
2. If a "Save As" prompt appears, proceed to step 4; if the prompt does not appear, proceed to step 3,
   
3. Ignore the screen image and select File/Save As from your menu bar,
4. Click the "OK" or "Save" button, or hit return,
   
5. Open the saved file from your word processor, now you can fill it out and print it.

Alternatively, call OTD at 6-3929 or email Lisa Darmo and we will send you a copy of the form via University or electronic mail.
